The HBPC APN (Advanced Practice Nurse) is Nurse Practitioner or Clinical Nurse Specialist.  The APN/Case Manager is a registered nurse with advanced educational and clinical preparation for the delivery of primary and comprehensive health care, incorporating selected medical services into professional nursing practice.  The APN/Case Manager provides comprehensive care to patients with a variety of medical, surgical or psychiatric diagnoses.  Care is provided to individuals, families and groups with an emphasis on the promotion of health and the prevention of disease.  The APN/Case Manager manages a panel of home care patients who have complex, chronic and end-of-life problems including common diseases and human responses to disease,  with a programmatic emphasis on cardiology/CHF and geriatrics.   The APN/Case Manager’s relationship with the physician is collegial with mutual referral and discussion, with the physician being ultimately responsible for all medical care rendered.  The physician will be readily available for medical consultation by the APN/Case Manager of the patient.   Along with the physician, the APN/Case Manager is accountable for the outcomes of his/her practice.  The schedule is Monday through Friday, day shift.

Applicants must not be listed on the Department of Health and Human Services (DHHS) Office of Inspector General Sanctions database.

Clinical privileges must be recommended by the Chief, Staff/Professional Standards Board and granted by the Director of Kansas City VAMC.  The provider must provide complete credentialing and privileging information in a timely, expeditious way to the Credentialing Office.   Employment is contingent on the ability of the provider to be credentialed by the VA Credentialing Office.
