The incumbent is assigned to the Office of the Nursing Service. The section, care line, or department description and patient population served is as defined in applicable Scope of Service. The Polytrauma Nurse Case Manager provides leadership that facilitates the delivery of quality nursing care for a group of patients through the utilization of the nursing process. This nurse works effectively with all levels of staff and promotes collaborative efforts with other healthcare professionals. The Polytrauma Nurse Case Manager facilitates and coordinates clinical, rehabilitative, and psychosocial services for patients who sustained complex and/or severe polytraumatic injuries. The incumbent demonstrates clinical competence in utilization of the nursing process during the provision and coordination of patient care. The incumbent functions as an effective member of a multidisciplinary healthcare team to plan, coordinate, and integrate appropriate admission, discharge, follow-up, health promotion, and rehabilitation services, referrals, resources, and communications among patients, family members, and the Veterans Health Administration (VHA).  The incumbent evaluates needs for specialized treatment and services, which may include clinical appointments, treatment, consultation, prosthetic fitting, education, support, discharge and follow-up care, and travel coordination. The Polytrauma Nurse Case Manager is responsible for the overall coordination and provision of comprehensive care and follow-up services for complex and diverse veteran populations.  Care will be provided in the least restrictive, most cost-effective manner, through use of the clinical practice guidelines and clinical pathways. Evidence of health promotion and chronic disease indices will be monitored through the clinical practice guidelines and performance measures. The Nurse III, Polytrauma Nurse Case Manager has programmatic responsibility for the VA nursing staff, services, and/or processes within specified area(s) of assignment. The Polytrauma Nurse Case Manager serves as the point of contact (POC) and coordinator for the Polytrauma Veteran Care Management Program and assumes all of the technical and administrative responsibility for the program management.  This nurse manages the daily operation of the facility Polytrauma Veteran Care Management Program and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfaction and organizational outcomes. The Polytrauma Nurse Case Manager functions as a registered nurse member of the multidisciplinary treatment team, and ensures the provision of comprehensive clinical services to the polytrauma veteran population. The Polytrauma Nurse Case Manager may encounter frequent complex and unpredictable caseload requirements. In addition, the incumbent may be required to travel from primary duty stations to various care and community sites.


KEY REQUIREMENTS:
