SCAN-ECHO (Specialty Care Access Networks-Extension for Community Healthcare Outcomes) In FY11, VHA made an organizational commitment to transform the delivery of specialty care services in order to improve access to specialists. One of the several initiatives to achieve this goal is SCAN-ECHO. SCAN-ECHO is modeled after Project ECHO - Extension for Community Healthcare Outcomes* which was developed at the University of New Mexico Health Sciences Center. Project ECHO allows for specialists to electronically connect with primary care providers in rural and remote locations. The model leverages telehealth, specifically by clinical videoconferencing equipment, to allow healthcare specialists from a regional health center to provide expert advice to providers in rural healthcare settings. Project ECHO has made it possible for patients with complex medical conditions such as diabetes, chronic pain, Hepatitis C, Rheumatoid arthritis, etc. to receive health care from their local providers without the inconvenience of traveling to a referral center. How does SCAN-ECHO work? A PCP at a CBOC or another VHA facility sends an electronic consult to the SCAN-ECHO Center. The Specialty SCAN-ECHO team (Specialists, Nurses, Pharmacists, Psychiatrists, etc) receives the consult and has an opportunity to review the consult request and the Veteran's medical record to determine appropriateness and to prepare a response. During the scheduled specialty SCAN-ECHO clinic, the PCP presents the Veteran's case using de-identified information. The specialist, as well as other members of the SCAN-ECHO team, will deliver a treatment plan and advice to the PCP. Other PCPs and PACT members not involved in the Veteran's care are able to listen in and participate in the discussion. During the SCAN-ECHO clinic, a didactic presentation is given discussing a topic for that specialty, which allows providers to earn CME/CEU credit.
