The Nurse Case Manager in Outpatient Mental Health Clinic is a vital member of the Mental Health Team who works in collaboration with licensed independent providers (LIP's) to assure high quality care to the veteran with psychiatric diagnoses/mental health needs.   The Nurse Case Manager works closely within an interdisciplinary team framework to help foster independent living through appropriate management of the client's mental illness by providing assessments, planning, interventions and evaluation of treatment.   Mental health treatment provided is  documented within prescribed guidelines in the Computerized Patient Record System (CPRS).   

He/She will report directly to the Associate Chief Nursing Service/Outpatient Mental Health.


KEY REQUIREMENTS:
  • U.S. Citizenship
  • Full unrestricted Licensure, Certification, or Registration required.
  • Must pass pre-employment examination.
  • Designated and/or Random Drug Testing required.
  • Background and/or Security Investigation required.
