JOB SUMMARY:

<tr><td valign="top" width="450"><b>Job Number 117211. Will perform complete physical examinations, including ordering, interpreting and evaluating diagnostic tests and examinations. Assume on-going responsibility for each patient from discovery of the disorder to recovery. Diagnose and treat both chronic and episodic disorders including complications of otherwise normal processes. Establish and document the health care plan and prognosis, maintaining records of each case that are sufficiently complete for any physician reviewing them to evaluate the effectiveness of the plan. Initiate drug therapy, taking responsibility for such therapy when performed prior to consultation with a physician. Determine when the patient has recovered from the disorder and releases the patient. Requires bilingual English/Spanish Level II-must pass Bilingual assessment test, graduate of an accredited RN and NP program, Masters degree, current California RN license, current California Nurse Practitioner Certificate, Pediatric Nursing certificate, DEA Registration number, California Furnishing number, American Heart Association BCLS, National Provider Identification number and approved Medicare provider, minimum 1 year experience. Must maintain current national certification from approved organizations-American Nurses Credentialing Center, American Academy of Nurse Practitioners, National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties. Prefer 3 of the last 5 years providing direct hands on clinical management of patients with complex CVS/CAD in Primary care ambulatory setting, face to face, telephone, email clinical patient management, case/care manager experience, knowledge of clinical strategic goals, PSP/VVP, POINT, Kaiser Mainframe, Health Connect system. Part time, 20 hours per week. Hours 830am-5pm. Scheduled days may vary. Hours may vary depending on patient care and department operational needs. Occasional travel off campus and weekends.
