<tr><td valign="top" width="450"><b>Affinity Nursing Services is recruiting Occupational Therapists for opportunities in Correctional Health for California Department of Corrections and Rehabilitation CDCR. Minimum of twelve 12 continuous months of experience within the last three 3 years performing the following services including, but is not limited, in a public or private institution. Internship does not count towards the required experience.
oProvide training to non-clinical staff in the special needs of mentally, physically
oand developmentally disabled patients
oDevelop individual treatment plans
oMaintain confidentiality of documentation
oParticipation in Interdisciplinary Treatment Teams
oAssess the patient-inmate capabilities, limitations, and/or observed behaviors, and provide appropriate therapeutic intervention
oPlan treatment goals providing specialized programs and services for specific patients
oDesign and maintain a written recreation activity schedule for identified patients
Must have the required licenses, permits, and/or certifications that are active and void of misconduct
oValid certificate as an Occupational Therapist from the National Board of Certification in Occupational Therapy
oA current State of California Occupational Therapist license with the Department of Consumer Affairs in order to perform services in the State of California.
Proficient in the English language and be able to communicate effectively must be able to speak, understand oral and written communication, and write effectively.
Correctional health experience is a plus.

All relevant work history including classification/job title, job duties performed, and any other information necessary to determine your qualifications.
If  you have provided services to any Correctional Health Facility previously, include the name of the specific Institution, to and from dates of service, and immediate supervisors full name and telephone number and
At least two 2 professional references and their contact information.
