The American Case Management Association places the role of Social Work in acute care hospitals in the realm of Case Management and describes the functions as follows:Case Management (Social Work) in Hospital and Health Care Systems is a collaborative practice model including patients, nurses, social workers, physicians and other practitioners, caregivers and the community. The Case Management process encompasses communication and facilitates care along a continuum through effective resource coordination.
