Provised education regarding home health services to all referral sources. Seeks and accepts referrals primarily from Memorial Medical Center (MMC) which provides the majority of the VNA referrals. The acuity of these patients supports the use of RN Liaison in evaluating these patients for admission to services.YOU ARE REQUIRED TO BRING YOUR CERTIFIED OFFICAL DMV PRINTOUT FOR EACH APPLICABLE APPLICATION YOU COMPLETE TO MEMORIAL
