This includes adherence to case management principles, assessment of patient needs, development of patient care plans, scheduling of patient and team assignments, administration of physical therapy care, evaluation of physical therapy care and supervision of non-professionals and para-professionals in the delivery of patient care in the home care setting while maintaining productivity standards. In this role, the Physical Therapist is responsible for positively impacting: clinical outcomes, patient care plan goals, appropriate utilization of the agency''s financial and human resources, and patient satisfaction. Physical Therapists who work a minimum of 72 hours per pay period will generally case manage 20-30 patients.
