<tr><td valign="top" width="450"><b>Title: Social WorkerCase Manager-Quality*Location: California-Central Valley-ModestoOther Locations:The Medical Social Worker participates in the assessment and case management of identified high-risk clients who are at a clear risk of needing a higher level of care due to a serious and persistent chronic condition. The Case Management Social Worker represents SGMF as an integral part of the Interdisciplinary Sutter Chronic Care Coordination Case Management Team. This position assists patient and families with little or no support system in place to access services and support. This position also performs on-going telephone monitoring for families in crisis to ensure support systems are in place and adequate. This position performs office, telephone and/or home assessments when needs are psycho-social in nature. This position works closely with the Nurse Case Manager to coordinate care to assure all patients'' need are met to provide optimal patient outcomes. This position requires travel to meetings and training throughout the SHCV Region as well as SH meetings held in locations outside of the SHCVR.Sutter Gould Medical Foundation (SGMF)is a non-profit, multi-specialty health care organization affiliated with Sutter Health. Based in Modesto, SGMF provides quality medical care, health education and research activities in a five-county area. SGMF''s medical staff includes family practitioners, internists and pediatricians, as well as a range of specialist physicians. Services include imaging, nuclear medicine, a health library, health education programs, occupational medicine, diabetes education, nutrition counseling, infertility counseling, and weight management.Masters in Social Work required with concentration in geriatric population preferred. L.C.S.W. required. Valid California Driver''s license. Typing certificate of 45 WPM required. Previous experience in community based health care. Experience developing care plans. Experience working as a member of a multi-disciplinary team. Experience in in-home and telephone patient case management, knowledge of health related information, education, resource and referral services. Knowledge of managed care. In-depth knowledge of geriatrics including common health & functional changes in the aged, and various chronic disease processes. Knowledge of the process of long-term care and the private/public resources available to support the clients served. Ability to work respectfully and creatively with clients of diverse functional abilities, social, economic, and cultural backgrounds to support both client and autonomy and client safety. Demonstrated skills in motivating, mentoring, coaching and redirecting clients in order to achieve compliance with various necessary medical, social and functional interventions. Ability to administer assessment testing and to create individual assessment protocols. Word processing and data base management skills. Standard office conditions, fast paced work, variable workload. Position requires frequent local travel throughout the greater SHCVR, periodically to areas with higher crime rates. Must have valid driver''s license and use personal vehicle in work related activities(mileage reimbursed at Sutter Health rates).
