Type: Regular Full TimeApply for this job:Your application choices are:* Apply for this jobAPPLY* Refer a friend to this jobMore information about this job:Overview:Under the direction of the PDS Management staff, and as a member of a trans-disciplinary team, provides assessment and intervention activities for children participating in developmental intervention services.Responsibilities:* Demonstrates the knowledge and skills necessary to assess and provide services, based on cognitive, psycho/social, educational, safety, and related criteria, appropriate to the age of the patients served in his/her assigned services area. * Facilitates and monitors activities in the classroom, clinic and other treatment areas according to previously approved treatment plan and daily lesson plans. * Nurtures appropriate behavior with children and makes decisions regarding needs for a behavioral consult or referral for additional services, such as SL, OT, PT and/or other agencies. She communicates all referrals to all appropriate parties (CM, pediatrician, etc.) including explaining the needs to the parents. * Participates in case review meetings and works closely with the CM to ensure that each child&rsquo;s needs are met * Primarily responsible for development of treatment plans and/or daily lesson plans (including preparation of future plans should coverage be needed due to an unplanned absence). * Provides individual (for Home Visits see &ldquo;g&rdquo; below) and/or group stimulation to the 0-5 age group, as appropriate for program. Child''s progress, parental feedback, and observation of sessions will be utilized for evaluation by supervisor. When conducting group stimulation for toddlers, such will be conducted in pre-school/childcare settings through mobile services, or as lead teacher for Community Toddler Enrichment Program. For the latter, duties will include: (a: Supervise classroom management, class routine and plans and upervises curriculum and activities.) (b: Participate in IFSP meetings and in the transition process.) (c: Perform and coordinates appropriate observations and assessment needed to monitor child and family progress.) (d: Responsible for compiling data, and completing paperwork.) (e: Maintain up-to-date records on all children participating in the Infant/Toddler Program.) (f: Monitors early intervention follow-up needs; provides parents techniques and strategies for at home stimulation.)  Provide Home Visits (schedules home visits, maintains regular communication and shares information with parents/caregivers; provides and models developmental intervention; monitors child and family progress in the home; completes required paperwork; assesses the need for, and requests consultations by specialists -- OT, PT, SLP, behavior &ndash; and assessed the home environment). * Supervises group management and routine. Provides play/language based intervention and implements DAP during sessions. Knows how to provide respectful accommodations to children across developmental levels. * According to the lead EI Supervisor&rsquo;s guidelines, complies, prepares, and develops materials for use in play-based, language stimulation sessions for implementing SLP recommendations resulting from consultations. * Actively monitors own schedule and keeps her schedule full and confirms appointments and fills empty slots. When conducting home visits, plans effectively for efficient use of resources. * Assists with assessments / screenings as needed when so required by specific circumstances (at various sites). Completes initial and ongoing language development related developmental assessment tools (devised for paraprofessionals). * Basic computer literacy ability to comply with department needs and expectations (i.e., documentation, obtaining background information and reports on patients, following up on appts, etc) * Completes required documentation
