    Provides professional nursing care in home setting using nursing processes. Act as case manager for the Home Health team in multi-disciplinary.
Essential Job Functions
     * Utilizes nursing process Assessments are thorough, complete and appropriately identifies patient needs and problems correctly. Establishes realistic care plan. Implements plan in appropriate and timely manner. Evaluates care and patients progress/lack of progress appropriately and plans for needed changes. Reassesses patient per home health policy.
     * Consult and update patients physician on patients needs and condition.
     * Act as case manager for multi-disciplinary cases.
     * Provides supervision to Home Health Aides.
     * Teaches and supervises patient, family and other caregivers taking care of patients.
     * Arranges referrals to appropriate agencies when need exists.
     * Completes patient documentation in a timely manner.
     * Completes necessary Administrative records as required.
     * Updates skills and keeps current in nursing knowledge.
     * Attends interdisciplinary team meetings when needed.
     * Fosters an atmosphere of teamwork and collaboration.
     * Participates in the Performance Improvement Program.
     * Maintain confidentiality in respect to all home health care matters.
     * Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned department by assessing, treating/caring for all patients physiological, psychosocial and learning needs to determine priority of implementation of age, appropriate nursing and/or medical interventions and teaching plan. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirement relative to his/her age specific needs and to provide the care needed as described in the departments policies and procedures. Age specific categories include pediatrics, adolescents, adults and geriatrics.
REQUIRED SKILLS
         * An RN who has graduated from an accredited school of nursing.
     * Must be currently licensed in Ohio.
     * Must have knowledge of venipunture and IV care.
     * Must have good communication skills and tolerance for varied lifestyles.
     * Must have current drivers license, automobile and auto liability insurance, which meets the Ohio financial responsibility laws requirements.
     * Must have CPR certification.
     * Demonstrate the ability to provide care to patients and family members of all ages, race and religious orientation.
Physical Requirements
     * Visual/hearing ability sufficient to comprehend written/verbal communication.
     * Ability to perform tasks involving physical activity In good physical health.
     * Ability to drive an automobile. 4. Ability to deal effectively with stress.
