<tr><td valign="top" width="450"><b>Job Details:* About us:MemorialCare Medical Foundation (MCMF) works closely with physicians and other health care professionals to provide best practice medicine to improve the health and well-being of individuals, families and our communities.Essential Job Outcomes & Functions:·Effectively identifies, evaluates, and addresses disease prevention and health promotion issues of the patient population by providing quality patient care guided by the protocols and standardized procedures.·Works in an independent and interdependent relationship with members of the Medical Director Team as well as fellow Nurse Practitioners which allows for consultation, collaboration and/or referral.·Provides individualized education to patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and/or medications.·Communicates with and provides records of patient treatment to primary care provider, referral source, and/or medical home.·Maintains legible, accurate, and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals and consultation.·Keeps accurate records for charts of all orders, education, counseling, evaluations and patient progress and recommended follow-up.·Facilitates evaluation of records by supervising physician(s) and peers to assure adherence to quality and standards according to the determined protocols and standardized procedures.·Participates in performance improvement and best practice activities to insure the delivery of quality patient care.
