<tr><td valign="top" width="450"><b>Medical CoderNovember 01, 2012 - November 30, 2012Location: Monterey, CAExempt/Non-Exempt: Non-ExemptEmployment Type: Full TimeDepartment: CHPDescription: Responsible for overseeing processing of clinic professional charges including updating of procedure and diagnosis codes in database coordinating reports and maintaining fee ticket files.Duties: ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties will be assigned and this Job Description will be changed from time to time by the Company. Responsibilities and procedures may be further described in a Policies and Procedures Manual.Assigns appropriate CPT-4 and ICD-9 codes to accurately report the physician s services provided to patients; Identifies physician services provided, but not adequately documented in the medical record.Evaluate encounter forms for correct EM levels, procedures and diagnosis codes; daily balancing of charges entered; charge entry correction for appropriate billing proceduresIdentify all significant diagnoses, conditions, and symptoms and ensuring each diagnostic or surgical code is provided with supporting documentationGathers, audits, corrects clinic fee tickets including checking for accurate patient demographic information and total charges by monitoring patient''s chart to verify ticket information.Works with medical staff to resolve coding issues and associated problems.Monitors Medicare reimbursements and maintains files. Assures information is available for peer and Medicare review.Participates in educational activities.Maintains strictest confidentiality.The job holder must demonstrate competencies applicable to the job position.Qualifications: EDUCATION and/or EXPERIENCEHigh school diploma or GED; minimum of two years of experience with accounting coding practices including one year of medical coding experience; knowledge of coding policies, procedures, and reimbursement practices; knowledge of health insurance processing and clinic operating policies.CERTIFICATES, LICENSES, REGISTRATIONSMust possess at least one of the following certifications: Certified Professional Coder (CPC) and/or Certified Coding Specialist (CCS)
