primary responsibilities are psychosocial
assessment, crisis intervention, brief therapeutic
interventions, complex discharge planning,
consultation, Selection representation, education,
community linkages and supportive services for
identified patients, families and staff both for
inpatient and outpatients. The services to be
covered are Living Liver Donors and the Living
Kidney Donors. The Live Donor social worker is the
dedicated Independent Living Donor Advocate ILDA
in the UCSF Medical Center Transplant Program. The
ILDA does not have any contact with liver and
kidney recipients and functions as the primary
advocate for all living liver and kidney donors.
The ILDAs role is to represent and advise the
donor, promote and protect the interests of the
donor and respect the donors decision and ensure
that it is informed and free from coercion.The
ILDA interacts with all potential living donors
during the evaluation phase and participates in
donor selection conferences. TheI LDA follows
donors through the transplantation and post-
transplant phases and sees all donors when they
return for post-transplant appointments. The ILDA
also follows up with living donors by phone at 6
months, 1 year and 2 years post-transplant to
obtain information required for UNOS follow-up and
to address any other questions or concerns the
donor might have as a result of their donation.The
LCSW coordinates resources and services in
response to patients psychosocial needs ensuring
timely discharge. The LCSW has advance knowledge
and skills in Clinical Social Work and is
distinguished from the MSW by leadership
activities in the department including
participation in advanced program planning,
provision of educational programs, clinical
supervision and consultation to the clinical team,
service lines, other departments and the
community.The LCSW provides age-appropriate
assessments, interpretation of data, and delivery
of interventions. Demonstrates the ability to work
with any patient regardless of race, gender,
religious affiliation, cultural beliefs,
lifestyle, disease process or treatment plan.
