<tr><td valign="top" width="450"><b>The ICM Coordinator provides clerical support and administrative coordination for the Intense Case Managers and acts as liaison between providers, ICM team and patients/families to facilitate continuity of care and referrals to appropriate community resources.
High School Diploma or equivalent required. Minimum of 2 years college education preferred. 2 years experience in the medical field, preferable in Home Health or discharge planning. Familiarity with medical terminology. Bi-lingual preferred.
