<tr><td valign="top" width="450"><b>The RN, Disease Manager provides nursing services and care coordination to high risk and/or high cost patients with chronic disease. Services are provided telephonically and on-site in the clinic. The RN Disease Manager provides care coordination, education, counseling and on-demand information to patients who have been enrolled in a condition specific program. This role allows highly skilled Registered Nurses to reach individuals with chronic diseases and influence their healthcare outcomes. The Disease Manager is responsible for coordinating certain operations of the Heart Failure/CAD/COPD Programs, including collaboration of services with the patient, physicians, nursing staff, case managers, social workers, home health infusion services, cardiac rehabilitation and the emergency department to facilitate management of the patient. In addition the position focuses on CAD patients, post Coronary Artery Disease Bypass patients, patients with Stents and hypertension and patients requiring medical management for CAD who are not surgical candidates. Strong Clinical background in Cardiac patients previous cath lab and/or Cardiac ICU experience desired. Knowledge of Disease Management and Case Management. Strong education skills to educate patients and families regarding disease process and medication.
Requires Registered Nurse licensed in California. Minimum of three years recent experience in any of the following cardiology, telemetry, critical care, home health, case/disease management. CCM desirable. Strong medical knowledge and competence in chronic disease pathophysiology, polypharmacy, clinical assessment and patient/family education. Excellent oral and written communication skills, conflict resolution, time management and negotiation skills. Basic computer skills required. Ability to work independently with a minimum of direction on a high volume caseload environment and deal effectively with rapidly changing priorities. Ability to anticipate/organize workflow, prioritize and follow through on responsibilities. Positive interpersonal, influencing and decision making skills. Ability to work constructively with a broad spectrum of health care professionals. Ability to work respectfully and creatively with clients of diverse functional abilities, social, economic and cultural backgrounds to support both client autonomy and client safety. No driving required. Must have BLS within 60 days of hire.
