<tr><td valign="top" width="450"><b>The Social Worker is a member of the multi-disciplinary team Case Management and Intense Case Management whose primary function is to assess the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these need and help provide both the counseling and casework needed to meet these goals.  This position independently evaluates patients psychosocial needs to promote quality care and cost-effective outcomes for patients requiring complex outpatient care.
Masters of Social Work MSW.  Minimum of 1 year of healthcare experience in social work.  Ability to develop plan of care based on the individuals specific medical, psychological, financial and other life issues.  Efficiently identify and resolve problems.  Understand insurance and community e.g., school or nursing home resources.  Must possess excellent telephone, computer and people skills, with an ability to interface with DOHC staff and outpatient providers in a pleasant and professional manner.  Negotiate with providers to achieve organizational goals and meet patient care requirements.  Respect and understand culture, confidentiality, legal and ethical issues that may affect care.  Current California driver license and proof of insurance.  BLS required within 60 days of hire.
