<tr><td valign="top" width="450"><b>Title: OUTREACH COORD - RNLocation: California-Southern California Service Area-San Bernardino-Community Hospital San BernardinoOther Locations: nullThe Outreach Coordinator in the Behavioral Healthcare Services setting performs duties within the scope of practice for his/her license as defined by California Department of Consumer Affairs. Behavioral Health Services professionals functions part of an interdisciplinary team per the Mental Health Department policies and procedures. They follow the guidelines of practice for the treatment of patients with psychiatric illnesses. The Outreach Coordinator performs the initial assessment of psychiatric patients to determine in collaboration with the psychiatrist what level of care is required for treatment. The Outreach Coordinator oversees the quality of outreach activities for the Department of Mental Health at CHSB. Nursing policies also address the range of practice for a registered nurse at this hospital. Interdisciplinary and Psychiatric Credentialing Committees regulate the practice of other Mental Health Practioners. This position requires the full understanding and active participation in fulfilling the Mission of Community Hospital of San Bernardino. It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support Community Hospital of San Bernardino strategic plan and the goals and direction of the quality improvement, process, and activities.REQUIREMENTS: Associates degree in Nursing is required. Alternative credentials may include LCSW, MFCC, Ph.D Bachelors degree in Nursing is preferred. Minimum of two (2) years psychiatric nursing or alternate professional role in an acute setting with responsibility for admission assessment, crisis intervention and mental status assessment. Current Ca RN Lic.,BLS and 51-50 certifications are required.
