JOB SUMMARY:

<tr><td valign="top" width="450"><b>PRINCIPAL RESPONSIBILITIES AND DUTIES
Initiate and develop new practice programs based
on the Physicians needs and facility
deficiencies. Coordinate practice programs with
appropriate company and hospital personnel and
departments for optimal outreach to specific
patient populations. Ensure appropriate data is
captured for scientific analysis to evaluate
patient/program outcomes. Assist the Physician by
obtaining patient histories and performing
physical exams, coordinate and schedule patients
for procedures, conduct pre-procedure medical
assessment of patients according to the
Physicians direction with minimal or no
supervision. Act as liaison between the
practices physician and referring physicians,
patients, and families. Coordinate all aspects of
patient care with the administrative and clinical
staff. Coordinate, review and analyze results of
diagnostic procedures, laboratory tests, and other
patient studies and medical reports, along with
communicating any significant findings to the
treating Physician. Provide appropriate education
to patients and their families regarding medical
findings and procedures.  Assist the physician in
procedures performed within the Facility under the
Physicians direction and as allowed under state
law. Perform procedures as specified in the scope
of practice under the supervision of the physician
and as allowed under state law. Follow patients
throughout their inpatient stay, outpatient visit
and post-procedure to ensure that appropriate
patient care, including testing, referrals, and
education are provided. Write prescriptions,
within the scope of practice, for appropriate
medication for patients as state regulations
allow. Participate in a variety of clinical and
educational conferences. Attend professional
meetings. Stay abreast of research literature.

Qualifications
PHYSICAL DEMANDS AND WORKING CONDITIONS
The physical demands and work environment
characteristics described here are representative
of those an employee encounters while performing
the essential functions of this job. Reasonable

functions. Day to day work includes desk and
personal computer work and interaction with
patients, facility staff and physicians. The
position requires travel between assigned
facilities and various locations within the
community. Travel to regional, Business Unit and
Corporate meetings may be required.The work
environment is characteristic of a health care
facility with air temperature control and moderate
noise levels. May be exposed to infectious and
contagious diseases/materials. Extensive mobility
required. Position requires ability to lift
objects weighing up to 50 pounds occasionally
perform difficult manipulative skills, arm-hand
steadiness, and hand-eye coordination skills walk
and stand for prolonged periods see objects
closely and hear normal sounds with some
background noise. Position requires ability to
concentrate on fine detail with constant
interruption and attend to tasks for 45 to 60
minutes at a time.
EDUCATION
Registered Nurse and graduate of an accredited
Nurse Practitioner program. Must have current
state licensure with national certification as a
Nurse Practitioner Must have an assigned National
Provider Number Masters degree in Nursing if
required by state law
EXPERIENCE AND REQUIRED SKILLS
Two or more years of experience in a health care
facility as a Nurse Practitioner Two or more
years of dialysis experience preferred Ability to
function independently with minimal supervision
Excellent verbal and communication skills
