<tr><td valign="top" width="450"><b>Title: Licensed Vocational Nurse - Care Coordinator Assistant - TemporaryLocation: San Francisco, CAUnder the direct supervision of a Registered Nurse (RN) or physician, in accordance w/ the legal scope of practice & w/in established standards of care, policies, & procedures, this team position performs significant member outreach activities to support the member''s ability to self-manage existing chronic or complex medical conditions (assists member in understanding & complying with the plan of care) and documents all activities to ensure the pilot study interventions can be evaluated for effectiveness. Demonstrates performance consistent w/ the Medical Center Vision, Mission, & Strategic Plan of the organization. Remains flexible to changing systems; is expected to demonstrate quality & effectiveness in work habits & clinical practice; & treats co-workers, patients, families, & all members of the health care team w/ dignity & respect.Essential Functions:-Utilizes effective communication strategies, including chain of command & issue escalation, which result in intended outcomes.-Performs patient care activities, w/in the scope of practice, considering age-related needs of the patient as assigned.-Reports unusual findings to the RN & contributes recommendations for revision to the plan of care.-Adheres to hospital standards, which customize care for age-related needs of the population served.-Keeps health care team members'' informed regarding patient''s responses to interventions.-Collaborates w/ RN to develop individualized plan of care that reflects clinical findings, planning, implementing, & evaluating the outcomes of that plan.-Collaborates w/ RN to assure completion of the discharge plan including discharge barriers & patient/family education.-Documentation reflects thorough data collection w/in the scope of practice, incorporates physical & psychosocial findings, diagnostic tests, learning needs, discharge barriers, multi-disciplinary education plans, & age appropriate care from admission through discharge.-Documentation reflects that the individualized teaching plan is followed; appropriate Multidisciplinary Patient Education Plans are implemented w/in the scope of practice.-Discusses patient findings & progress toward outcomes w/ RN, physicians, & other members of the health care team.-Practices customer service standards as defined by the Service Area, Medical Center, & specified dept.-Anticipates the customer''s desires, takes proactive steps to meet those needs, by listening & taking responsibility w/in scope to ensure issues are resolved.-Assists members who are less than satisfied w/ their experience by listening & taking responsibility w/in their scope to ensure that the issue is resolved.-Under the direction of the RN, gives patients & their significant others the information needed w/in their scope of practice.-Makes appropriate referrals under the direction of the RN & facilitates the customer''s ability to utilize resources.-Maintains & protects patient confidentiality.-Acts as a resource, preceptor, & mentor to new employees, registry, students, & other team members.-Perform other duties as specified in full job description.-This job has no supervisory responsibilities.Qualifications:Basic Qualifications: - One year clinical experience (within the past four years) required.- Three or more years experience (within the past five years) working with chronic conditions populations preferred.- Experience in automated clinical information systems preferred.- High School Diploma/GED.- Current California Vocational Nurse License.- California Drivers License.- BLS Certification.- Knowledge of Kaiser Permanente medical center services policies and procedures.- Excellent verbal and written communication skills and interpersonal skills.- Working knowledge & proficiency in using Word, Excel or Access software, & Health Connect required or willingness to take a training course prior to, or w/in 30 days, of employment i
