<tr><td valign="top" width="450"><b>The Medical Record Review Abstractor in San Francisco performs HEDIS medical record over read on retrospective delivery of care related to chronic disease and preventive care previously abstracted from a clinical team with goal of achieving 100% accuracy related to HEDIS certification audit. Duties include: * Enter aberrant findings into an access-like data base and discuss results with constructive feedback to clinical team* Perform medical record abstraction, & enter findings into database* Identify opportunities & track progress* Call provider offices & hospitals to request medical records* Call members to identify date & provider of HEDIS service* Data enter call results in database* Coordinate with Collector and manage caseload* Communicate with team members to effectively manage project assignment* "Over Readers" will have additional responsibilities but with same basic requirements
