The Admissions Nurse is responsible for conducting an initial nursing assessment of a potential hospice patient and reporting the patient''s condition to the Hospice Medical Director using the applicable Local Coverage Determinations. If the Hospice Medical Director supports the hospice diagnosis and gives verbal certification of terminal illness, the Admissions Nurse will admit the patient to hospice services and attend to the immediate needs of the patient. The Admissions Nurse will document the assessment, interventions and outcomes in the patient''s chart utilizing the electronic medical record at the point of care.Education:Graduate from an accredited school of nursing with a current California Registered Nursing license. California Public Health Certificate or completion of Community Health Nursing program preferred.Experience:Minimum one year Hospice experience. Experience with admissions or re-certifications under Region IX Local Coverage Determinations (LCDs) strongly preferred. Knowledge:Possess critical thinking skills, in-depth knowledge of clinical disease entities common to hospice and palliative care for terminally ill patient. Possess knowledge of the hospice philosophy, the Medicare hospice benefit, Conditions of Participation, and Local Coverage Determinations under the Medical Fiscal Intermediary (FI). Should also possess knowledge of hospice reimbursement methods both under government benefits and commercial payors.
