Our relationship with this outstanding medical center allows us to extend the practices of primary and secondary care physicians by offering expertise for complex cases, leading-edge technology and the highest standards for patient care.Primary Purpose:Under the clinical supervision of the Anticoagulation Clinic Medical Director and the administrative supervision of the clinic Manager of the Family Health Center, the incumbent will perform specialized nurse practitioner duties in the clinical outpatient setting. These functions include collaborative management of new and follow-up patients in the Anticoagulation Therapy clinic.This position combines advanced nurse practitioner knowledge of physical assessment and treatment under the supervision of a physician with skills in case management, education and research. This position is responsible and accountable for the quality of care within the scope of the position in accordance with standardized procedures for patients with cardiac and/or other related clinical disorders. The incumbent assesses patient needs as well as plans and directs approved educational programs. The designee also manages patients on a case by case basis by assessing, diagnosing, coordinating, educating and documenting pertinent information to meet patient care, hospital and national cooperative group guidelines. The nurse clinician plays an integral role in the patient care team acting as a resource and educator to patients, family members and clinical staff.JOB PERFORMANCE STANDARDS: (Principal Accountabilities)I. Service Standards: Consistently exhibits Service Standard behaviors set forth in the attached Service Performance Standards (see attached listing).II. Essential Functions: Utilizes skills specific to the Nurse Practitioner to assess, diagnose, plan and implement the anti coagulation medical care of patients in collaboration with a physician. Utilizes advanced learning skills to develop, implement and communicate a plan of care for inpatient and outpatients. Utilizes clinical knowledge to assess, plan, implement and evaluate an educational program for patients, families, and staff. Supports program promotion and outreach programs.III. Critical and Analytical Thinking:Collaborates with physician and assists with management of anticoagulation therapy with selected clientele. Plans, coordinates, delivers and evaluates anticoagulation therapies based on pre-established guidelines and protocols. Provides interpretation of data and diagnostic results (INR and targets).
