<tr><td valign="top" width="450"><b>Maintain and update resident profiles in diet office information system Geri-Menu and binders interview residents for diet history, food and meal preferences for nutritional assessment and meal preferences/satisfaction.  Also, participate in quarterly/annual care plan assessment.
Essential Functions
Maintain and update Master Menu. Review with Certified Dietary Manager and Executive Chef regarding changes and diet modifications
Interview and screen new residents for diet history, food and meal preferences, menu satisfaction, and meal acceptance and dining related needs. Contact family members, as needed, to complete initial nutritional assessment and interview. Refer to Clinical Dietitian residents at high nutritional risk or residents who need further assessment or follow up. Enter data regarding food preferences or food allergies in Geri-Menu
Participate in quarterly/annual care plan assessments, make quarterly/annual progress notes as assigned, and other interdisciplinary meetings as appropriate or assigned by Clinical Dietitian
Perform nutrition education for residents as appropriate
Assist residents with menu selections in accordance with therapeutic diet considerations and reviews food preferences with residents as needed
Communicate with Food Service Director, Executive Chef, and Nutritional Services supervisors concerns related to residents food preference, meal preparation and services. Assists with kitchen supervision as assigned.
Participate in Nutritional Services Department staff meetings
Perform other duties as assigned
