There is currently a vacancy for a full time IV certified Practical Nurse as part of a Patient Aligned Care Team in the Primary Care Clinics. As part of the clinic staff, the LPN will be working a day shift, Monday through Friday. The individual in this position works under the periodic supervision of a professional nurse and is responsible for the nursing care provided to the patients assigned to him/her. The LPN performs such tasks as preparing and administering prescribed treatments and medications; identifying and interpreting deviations in patients normal state of health to initiate nursing actions as appropriate; recognizing patient/family teaching needs to provide information or seeking out resources to meet those needs; providing and documenting health teaching conducted. Must be able to work effectively with patients/families, physicians, other professionals, and support personnel in accomplishing required tasks/assignments. This individual may be assigned to be primary support person to selected Primary Care Providers and may occasionally be reassigned to cover other areas. The LPN will be under the supervision of the Nurse Manager for the assigned clinic.
This announcement is open until filled. First referral to selection manager will be made after September 27, 2012. Subsequent referrals will be made, as needed, until position is filled.

Salary and grade will be determined upon qualifications and experience.
