JOB SUMMARY:
Social Service/Admissions Director Must have Social Workers License. Experience is preferred Duties: Complete Social Histories. Complete MDS sections as appropriate. Participate as member of Interdisciplinary Team in preparing care plans. Document all conditions, attitudes, responses of residents quarterly or as needed. Serve as liaison between facility, residents, family, responsible parties and outside agencies. Provide social and psychological support for residents. Complete admissions and paperwork, and entire admission process. Meet with administration, medical and nursing staff and other related departments in planning social services. Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health and welfare and social agencies to ensure that social service programs can be properly maintained to meet the needs of the patient/resident. Implement policies and procedures for the identification of medically related social and emotional needs of the patient/resident. Participate in discharge planning; development and implementation of social care plans and patient/resident assessments. Refer patient/resident families to appropriate social services agencies when the Facility does not provide the services or needs of patient/resident. Provide information to patient/resident/families as to Medicare/Medicaid, and other financial assistance programs available to the patient/resident. Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to best meet the needs of the patients/residents and families. Maintain confidentiality of all pertinent patient/resident care information to ensure patient/resident rights are protected. Complete tours using the four closing questions. Attend Medicare and Census meetings. Perform other duties as assigned by the supervisor. Develop, post, and implement monthly specific planned psychosocial and behavior programs by following facility policy and procedure. Document all conditions, attitudes, responses, etc. of the resident towards programs in which he/she participated by following facility policy and procedures. Wage will depend on experience.
