The Department of Veterans Affairs Medical Center, Cincinnati, OH is currently recruiting for One (1) Nurse Practitioner (NP), for the Hospital in Home Program based in Hamilton, Ohio.
One (1) full time positions is open for a Nurse Practitioner for the Hospital in Home Program in Primary Care Service. The Nurse Practitioner is an integral part of the interdisciplinary team providing histories and physical exams, planning care, evaluating outcomes, revising care interventions and providing care coordination. The NP will serve as a Hospital in Home consultant to various staff, disciplines and services within the medical center and community. The NP provides nursing leadership within the program and will participate in educational programs that lend an opportunity to share expertise and knowledge in care. The Hospital in Home Program is based in Hamilton, Ohio CBOC. The NP will report to the Nurse Manager, HBPC. This program requires travel to patients homes within a 30/50-mile radius of the Medical Center.
VALID DRIVER'S LICENSE: You must have valid driver's license as the incumbent will drive a government car.
WORK SCHEDULE: Monday through Friday, 8:00 am - 4:30 pm with weekend coverage as needed.
