The VA Roseburg Healthcare System (VARHS) consists of one Veterans Health Administration (VHA)facility located in Roseburg, OR, and four Community Based Outpatient Clinics(CBOC) strategically located Eugene, North Bend, and Brookings, OR and Crescent City, CA.    VARHS offers primary care and hospital services in medicine, surgery, and mental health for the 62,000 veterans who reside in Central and Southern Oregon and Northern California

• 50% or Greater
• Provider will be required to make home visits to HBPC patients. GSA vehicle will be used if available. Otherwise personal vehicle must be used with appropriate reimbursement for mileage.
