Caregiver Support Nurse

The Caregiver Support nurse is responsible and accountable for the initial and continuous assessment in-the-home assessment for a culturally diverse veteran patient population.  This nurse evaluates, plans, implements, and coordinates care needs, conferring with the Caregiver Program care team to deliver care in the home setting.  This individual provides teaching to the patient and caregiver regarding ways of managing and delivering nursing care in the home; monitors patient's condition and assess care levels; provides needed assistance and consultation for home care referrals. 

TRAVEL REQUIRED

• 50% or Greater
• Travel may extend beyond normal adminstration hours of 8am to 4:30pm
