JOB SUMMARY:
Rocky Mountain Care Blanding is seeking a full/part time licensed Social worker. The successful candidate will be responsible for hospice and home care psychosocial support for patients and care givers, educating the community on services, patient discharge planning, and various tasks/ event planning. Must have current social work license, driver's license, and home care hospice experience preferred. Must have own car (mileage reimbursement provided). Excellent time management and great people skills are essential for success in this position.
