JOB SUMMARY:
Functions as a Homecare employee within health care facilities to enhance the transition of the client to Homecare services once patient choice has been made. Performs assessment of the needs of referred patients by determining appropriateness for admission, assisting in obtaining authorization, and coordinating services and supplies. Works in conjunction with the health care team to coordinate care. Functions as a resource to the health care team. Minimum Requirements Current Registered Nurse Licensure within the state of practice Three years Health Care Nursing experience One year of Homecare experience Basic computer skills Excellent customer service skills must be able to work independently, and prioritize and perform detailed work in a fast-paced team environment Excellent communication skills Physical Requirements Walking, Standing, Listening, Speaking, Manual Dexterity Preferred Qualifications Bachelor s degree Insurance, or other third-party payer experience.
