IMMEDIATE NEED. FULL-TIME OR PART-TIME REGISTERED NURSE / CASE MANAGER for a healthy, growing, accredited Home Health and Hospice provider. Employer is looking for candidate for RN to work from their South Ogden office, providing exceptional care to patients in their home environment. Care includes comfort and aid to terminally ill patients, and also to those needing home-care rehabilitative services. Utilize your clinical skills in providing for the medical, personal, and physical needs of the patient. Demonstrate excellent communication and reporting skills in the timely documentation of patient care. Oversee and coordinate the work of other team members in providing patient care Requirements * RN License. * CPR certification. * Home Health and/or Hospice experience required. * 3-5 years of successful RN / Case Manager experience. * Exceptional communication skills are a must!! * Valid Utah drivers license. * Insured and reliable transportation. * Highly developed organization skills and attention to detail. * Ability to fully evaluate, plan, and monitor patient care. * Compliance with accepted professional standards and practices.
