               <td align="left"><font style="font-size: 100%;">Social Worker<BR>Department: Care Management<BR>Schedule: Part-Time<BR>Shift: Days<BR>Hours: 8<BR>Hours/Pay Period: 48<BR>Job Details: <BR>* <BR>We are currently seeking aper diem/on-call Social Workerto work on an as-needed basis on the day shift, including weekends and holidays. This position provides direct services to patients, families, and staff as part of an interdisciplinary team. Areas of coverage may include any or all of the following: Med/Surg, Critical Care, Mother/Baby, Oncology, Orthopedic, and Psychiatry. You are responsible for patient clinical assessment and treatment recommendations, along with coordinating resources to provide after hospital planning, identification/resolution of problems related to patients'' transitions within the health care setting, after hospital planning, psychological support or crisis intervention and appropriate utilization of organizational resources including appropriate length of stay.<?xml:namespace prefix = o ns = "urn:schemas-microsoft-com:office:office" /><BR>Qualifications<BR><BR>?    Requires BSW from anaccredited university, with MSW<BR>?    Must possess one of the following licenses from the state of Washington: Licensed Independent Clinical Social Worker (LICSW), Licensed Advanced Social Worker (LASW), or be eligible to register as an Agency Affiliated Counselor upon hire<BR>?    Should also have familiarity with utilization management criteria and broad knowledge of community resources, medical terminology, hospital systems, insurance guidelines, and financial and risk management issues<BR>?    Advanced degree and/or ASHP accredited residency or equivalent experience preferred.
