               <td align="left"><font style="font-size: 100%;">Title: Registered Nurse Admissions - Home Health<BR>Location: Washington-Everett<BR>Job Number:_10672<BR>Providence  is calling a Home Health Registered Nurse Case Manager to Providence Hospice & Home Care of Snohomish County in Everett, WA.<BR>We are seeking an experienced, independent and compassionate RN to provide quality care to patients and their families in the home.<BR><BR>In this position you will:<BR>* Drive throughout your work day to see patients in private residences, assisted living, and skilled nursing facilities within a designated region<BR>* Assess, plan, implement and evaluate nursing care for the patient utilizing the interdisciplinary care team<BR>* Demonstrate critical thinking skills, decisive judgment and the ability to work with minimal supervision<BR>* Utilize software programs for the maintenance of patient records and communication with team members<BR><BR>Qualifications:<BR><BR><BR>Required qualifications for this position include:<BR>* Graduate of an accredited school of nursing<BR>* Current Registered Nurse (RN) licensure<BR>* Current driver?s license and insured, reliable vehicle <BR>* Two (2) years nursing experience  <BR>Preferred qualifications for this position include:<BR>* Bachelor?s Degree in Nursing (BSN)<BR>* Current WA State RN license<BR>* Current WA State driver''s license<BR>* Current CPR certification<BR>* Experience in a Medicare certified Home Health or Hospice agency<BR>* Experience with Microsoft Office software programs including Outlook, Word, and/or Excel <BR>* Strong computer keyboarding skills and experience working with an Electronic Medical Record (EMR) system <BR><BR>About the ministry you will serve.<BR>Providence Hospice and Home Care provides quality, timely, comprehensive medical care in the homes of patients in Snohomish County in Washington State, including Camano Island.
