<td align="left"><font style="font-size: 100%;">Title: Registered Nurse Case Manager - Hospice<BR>Location: Washington-Everett<BR>Job Number:_18367<BR>Providence is calling a Registered Nurse Case Manager to Providence Hospice & Home Care of Snohomish County in Everett, WA.<BR>We are seeking a compassionate and caring RN to ensure that patients and their families receive quality, cost-effective health care.<BR><BR>In this position you will:<BR>* Assess and monitor patients, determine eligibility for treatments and ensure the plan of care meets the patient''s needs<BR>* Ensure that the resource utilization, care delivery, and discharge planning are closely coordinated so as to optimize the patient''s stay in the hospital <BR>* Contribute to the clinical performance improvement by providing direction on ways to improve efficiencies, reduce costs, receive appropriate reimbursement, and meet regulatory requirements<BR><BR>Qualifications:<BR><BR>Required qualifications for this position include:<BR>* Graduate of an accredited school of nursing<BR>* Current Registered Nurse (RN) licensure<BR>* Current driver''s license and insured vehicle<BR>* Three (3) years of nursing experience<BR>Preferred qualifications for this position include:<BR>* Bachelor of Science Degree in Nursing (BSN)<BR>* Current CPR<BR>* Current WA State RN license<BR>* Current WA State driver''s license<BR>* Experience in hospice, home health, oncology, medical/surgical, skilled nursing, long-term care<BR>* Proficiency with electronic medical records<BR>* Proficiency with MS Office Word and Outlook<BR>About the ministry you will serve.<BR>Providence Hospice and Home Care of Snohomish County provides quality, timely, comprehensive medical care in the homes of patients in Snohomish County in Washington State, including Camano Island.
