               <td align="left"><font style="font-size: 100%;">Title: Social Worker MSW - Hospice<BR>Location: Washington-Everett<BR>Job Number:_18369<BR>Providence is calling a Hospice Social Worker to Providence Hospice and Home Care of Snohomish County in Everett, WA.<BR>We are seeking a licensed social worker to provide hospice services to patients, families and caregivers that address a wide range of psychosocial needs related to end-of-life care.<BR><BR>In this position you will:<BR>* Promote patient and family choice, assuring continuity of care, and enhancing the quality of life of dying patients.<BR>* Develop and implement a plan of care in collaboration with the Hospice Interdisciplinary Group (IDG).<BR>* Address psychosocial issues that may include: adjustment to illness/prognosis, care system, community resources, insurance/financial, legal/ethical, and end-of-life planning.<BR>* Use your exceptional communication skills to help patients and families deal with the impact of end-of-life planning.<BR><BR>Qualifications:<BR><BR>Required qualifications for this position include:<BR>* Master?s degree in Social Work from an accredited school<BR>* Current Social Work Certification or License through Washington State Department of Health<BR>* Current CPR certification<BR>* Valid driver?s license and daily access to a reliable, insured vehicle<BR>* One (1) year post master''s work experience<BR><BR>Preferred qualifications for this position include:<BR>* Prior social work experience in a hospital, hospice or palliative care setting<BR><BR><BR>About the ministry you will serve.<BR>Providence Home and Hospice Services provides quality, timely comprehensive medical care in the homes of patients and with facilities in Washington State?s King, Snohomish, Thurston, Mason and Lewis Counties.
