The Social Worker is responsible for coordinating, evaluating, and monitoring the plan of care in collaboration with the attending physician/medical director, care management, and the nurse co-casemanger. The Social Worker assists patients and families in identifying and coordinating community resources to enable optimal comfort and safety, address psychosocial issues in order to meet end-of-life and quality of life goals.? The Social Worker maintains current knowledge of community resources. The Social Worker works in a fast-paced hospital environment.<BR>Qualifications and Requirements Education Background Graduation from an approved master?s or doctoral level social work program The American Association of Social Work Boards (ASWB) exam is required for licensure  The advanced exam is required to be a LASW The clinical exam is required to be a LICSW<BR>Work Experience One-year hospice social work experience. Previous work in a hospital environment strongly preferred.<BR>Motor Vehicle Experience/Record All drivers must be 18 years of age and have been driving for a minimum of two (2) years, although 21 years of age is preferred.? All drivers must have a current, valid<BR>Washington State Drivers License and current auto insurance.??  License or Certification Licensed Registered Counselor in the State of Washington. Current Washington State driver?s license and proof of insurability.
