Evaluate psychosocial assets and deficits for the purpose of enabling the pt to maintain the greatest level of independence. Coordinates with others to establish a plan of care. Provides short term counseling, teaching and brokering with other resources for care giving needs.<BR><BR>This position will work in an urgen care setting. Candidates with experience in urgent care or emergency rooms will be preferred.<BR>Education<BR>Minimum Education Requirements<BR>Masters Degree in Social Work (MSW).<BR><BR>Preferred Education<BR>Unspecified <BR><BR><BR>Knowledge/Skills<BR>Minimum Knowledge/Skills Requirements<BR>Excellent written and verbal communication skills. Demonstrated success as part of a multi-disciplinary team and commitment to diversity.<BR><BR>Preferred Knowledge/Skills<BR>Intermediate knowledge of social case management, conflict resolution and team building practices. Familiarity with Microsoft Office. Familiarity with electronic charting.<BR><BR><BR>Experience<BR>Minimum Experience Requirements<BR>1 year Previous social work experience<BR><BR>Preferred Experience<BR>2 years Post-Masters experience preferably in an urgent care or emergency room setting.
