               <td align="left"><font style="font-size: 100%;">Family Home Care, the largest Home Health Agency in Spokane County, has an opening on our team for an on-call Medical Social Worker for our Spokane Branch.  <BR><BR>The Medical Social Worker must provide skilled, creative social work services to a caseload of clients and their families within the home environment.  This includes counseling for long term planning and decision making.  The MSW must provide skilled psychosocial assessment therapy, short-term therapy, grief counseling, crisis intervention and assists clients and families in accessing appropriate community resources.<BR><BR>The MSW will serve as a provider of social work services utilizing the social work model in collaboration with other members of the healthcare team.  The MSW may provide services to clients of all age groups including neonates, infants, pediatric, adolescent, adult and geriatric patients who are physically or mentally ill, disabled, injured or terminally ill with various cultural needs and issues. The MSW must possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs, and to provide the care needed.  <BR><BR>This position will work with both the Medicare certified Home Health department as well as the Hospice Department.  The MSW will assess and deliver services to Home Health patients as part of an interdisciplinary team delivering care over a period of time on an intermittent basis.  For the Hospice department, the MSW will provide support to families and patients who are terminally ill.  This includes counseling and grief support.  The MSW will also direct the bereavement program for the Hospice Department according to the Medicare Hospice benefit and conditions of participation.</font></td>
