To those who embrace the spirit of these words and our commitment to Exceptional Medicine and Compassionate Care, we offer the opportunity to learn and grow as a member of the PeaceHealth family.<BR>Job Description:<BR>JOB SUMMARY<BR><BR>Performs therapeutic interventions, participates with the Behavioral Health Team in developing patient care/treatment plans, assessments, discharge planning; case management and coordination of care.<BR><BR>ESSENTIAL FUNCTIONS<BR>1. Completes appropriate intake documentation, provide assessment, develop treatment plan( interprets social situations as related to patient??s condition, hospitalization and post-discharge needs to modify treatment plan accordingly), manages cases, assesses community resources, monitor patient to regularly adjust treatment plan to suit patients needs and develop discharge plans. Maintains complete documentation on patient throughout the process and ensures proper authorizations are in place.<BR>2. Provides individual and/or group sessions with patients, families or both as requested by physician or treatment team to address issues identified in social history that affect treatment and discharge.<BR>3. Collaborates as appropriate with care providers in the treatment and discharge planning, participates in treatment team meetings to assure provision of appropriate social services to patients while maintaining compliance the confidentiality/privacy laws.<BR>4. Collects data, reports on statistics or data as appropriate for leadership. Assists with monitoring and evaluation of services provided for department quality improvement plan and development of programs/service plans as requested by leadership.<BR>5. May assist with making alternate living arrangements, home care assistance, and or coordinating referrals to external community resources. Attends relevant intra/inter agency meetings as directed by leadership. 6. Communicates appropriately with the person served regardless of their age.<BR>7. Performs other duties as assigned.<BR><BR>QUALIFICATIONS<BR>EDUCATION:<BR>* Bachelor Degree in Behavioral Health Sciences or related field from a college or university.<BR>EXPERIENCE/TRAINING:<BR>* Minimum of three years of related experience preferred.<BR><BR>* Understaning of all major treatment modalities, including Solution Focused Therapy, DBT, and Cognitive Behavioral Therapy and is able to employ them appropriately.<BR>LICENSE/CERTIFICATION:<BR>* WA State: Registration: Must </font></td>
