               <td align="left"><font style="font-size: 100%;">Care Transition Advocate (CTA)<BR> <BR>Department Care Transition Advocate<BR> <BR>Location HRC 9D60<BR> <BR>Work Unit Description <BR> <BR> <BR> <BR>Work Schedule, Hours, FTE, Salary Range <BR> <BR> 1.0 FTE; Monday - Friday, 8:00am - 5:00pm but shifts may vary as needed, may require on call responsibility, availability by pager and/or cell phone. Location: Marquam Hill.<BR> <BR>Functions/Duties of Position <BR> <BR> The Care Transition Advocate will implement improvements in care coordination from hospital to home and provide patient education and coaching during the hospital and post-discharge time period for underserved patients admitted to OHSU.<BR> <BR>Job Requirements <BR> <BR> Required: MSW, Bachelor?s Degree in Nursing or other combination of education, experience and training in healthcare discipline that provided the opportunity to manage care coordination for various patient populations.3+ years of experience; applicants with strong medical background and with experience working with adult special needs populations, including but not limited to community mental health, chronic illness, complex social issues. Experience working with an adult underserved, chronically ill population and familiarity with common outpatient medications and general medical knowledge. Strong communication (written and verbal) and organizational skills; works well as part of a multidisciplinary team, is pro-active, flexible and a problem-solver. Valid OR/WA driver?s license with an acceptable driving history and access to a car on a daily basis for home visits. Preferred: 5+ years of experience; applicants with strong medical background and with experience working with similar adult populations (community mental health, chronic illness, and complex social issues). Experience working in Portland area safety net. Experience doing home visits, community outreach. Training in motivational interviewing, health literacy, and trauma-informed care. Experience with continuous quality improvement. LCSW or CSWA in the state of OR; RN or other relevant credentials that support job requirements.<BR> <BR>Additional Details <BR> <BR> The CTA will see patients both in the hospital and in partnering clinics, and may do community/ home outreach.
