               <td align="left"><font style="font-size: 100%;">Title: Case Manager- Maternity Services<BR>Location: Oregon-Portland<BR>Job Number:_12890<BR>Providence is calling an on-call Case Manager- Maternity Services for a varied day shift position with some weekends, in clinics and hospitals in the Portland service area in Portland, OR. <BR><BR>In this position, you will:<BR><BR>* Provide case management to high risk maternity patients across the continuum of care. Case managers are assigned to cover outpatient clinics and the hospital maternity and neonatal intensive care units. The maternity case manager, in conjunction with the patient and caregivers, develops, implements, and monitors, a plan of care to support positive birth outcomes and successful transition to parenting.<BR><BR>Qualifications:<BR><BR><BR>Required qualifications for this position include:<BR>* Masters degree from a School of Social Work accredited by the Council on Social Work or equivalent or; equivalent experience typically required working in Case Management as an RN. <BR>* Five years of relevant clinical, medical/and or psychiatric experience.<BR><BR><BR><BR><BR>Preferred qualifications for this position include:<BR><BR>* One year clinical experience within PHS of Oregon.<BR>* Experience or training in crisis intervention, solution-focused therapy, and motivational interviewing.<BR>* Spanish speaking.<BR>About Providence in Oregon<BR><BR>As the largest health care system and largest private employer in Oregon, Providence offers exceptional work environments and unparalleled career opportunities.<BR><BR>The Providence Experience begins each time our patients or their families have an encounter with a Providence team member and continues throughout their visit or stay.
