               <td align="left"><font style="font-size: 100%;">Title: Consult Liaison Services Coordinator<BR>Location: Oregon-Portland<BR>Job Number:_17328<BR>Providence is calling an on-call Consult Liaison Service Coordinator for a day shift position at Providence St. Vincent Medical Center in Portland, OR. <BR><BR>In this position, you will:<BR><BR><BR>* Ensure patients with behavioral health issues receive the proper care and treatment while they are in the hospital on medical/surgical units and/or the ED.<BR>* Coordinate the tracking system and work with other professionals within the hospital to ensure this goal is met.<BR>* Require excellent clinical and program development skills in order to ensure that systems are in place to provide evaluations, treatment planning, staff development and disposition planning for these patients.<BR>* Be responsible for providing assessments and clinical interventions for wide array of patients, as part of the job responsibilities.<BR>* Also be responsible for either providing staff education &/or arranging for education regarding specific skills in working with patients with behavioral health issues, especially skills in disposition management.<BR><BR>Qualifications:<BR><BR><BR><BR><BR><BR>Required qualifications for this position include:<BR><BR>* MSW or equivalent experience typically acquired working in Case Management as an RN.<BR>* Coordinator must have at least 5 years experience in organizing the care of patients in a variety of settings with varied diagnosis.<BR><BR><BR>Preferred qualifications for this position include:<BR><BR><BR>* CNS certification or Licensure by appropriate Board.<BR><BR>About Providence in Oregon<BR><BR>As the largest health care system and largest private employer in Oregon, Providence offers exceptional work environments and unparalleled career opportunities.<BR><BR>The Providence Experience begins each time our patients or their families have an encounter with a Providence team member and continues throughout their visit or stay.
