<td align="left"><font style="font-size: 100%;">Hepatology Nurse Coordinator - Patient Coordinator<BR> <BR>Department Digestive Health Center<BR> <BR>Location PPV 33089<BR> <BR>Work Unit Description <BR> <BR> Digestive Health Center, CHH 6th floor<BR> <BR>Work Schedule, Hours, FTE, Salary Range <BR> <BR> 1.0; Schedule: Mon-Fri, 7:00am-7:00pm, variable; Location: Center for Health & Healing, South Waterfront.<BR> <BR>Functions/Duties of Position <BR> <BR> The Nurse Coordinator is a member of a multi-disciplinary team with primary responsibility for direct and indirect professional nursing care of medical patients that have Hepatology disorders and diseases. S/he will provide clinical support services to physicians in the Hepatology Practice. The Nurse Coordinator will apply nursing knowledge, critical thinking and clinical judgment to ensure patient?s right to receive appropriate care. Across all programs, s/he will be responsible for development and delivery of patient education, assessing patient and family needs, developing nursing care plans, implementing and evaluating the care delivered. This position will coordinate follow up care for patients as recommended by the Hepatology care team. This position will be the patient navigator throughout the OHSU and other HealthCare facilities as necessary. The nurse coordinator will assist with monitoring diagnostic results and the patient care progress/processes as requested by providers. The Nurse Coordinator is considered a specialist with in-depth knowledge of Hepatology disease processes. The Nurse Coordinator is expected to practice in accordance with professional standards, core values, and with a commitment to service excellence.<BR> <BR>Job Requirements <BR> <BR> Current Oregon nursing license. Current BLS certification issued by the American Heart Association. One (1) - two (2) years experience in Medical/Surgical nursing. Ability to function as a contributing team member. Willingness to learn. Documented positive evaluations by management. Experience with EMR system. Experience with creating and the delivery of patient education. Preferred: Bachelor?s degree. Specialty Care Management of Hepatology patients. Knowledge of GI/Liver disease surgical and medicine strongly preferred. Ambulatory Nursing strongly preferred. Experience working and coordinating in a multidisciplinary practice model; Knowledge of EPIC EMR favorable.<BR> <BR>Additional Details <BR> <BR> Ability to work Monday thru Friday varied hours. Exposure to body fluids- must use proper PPE (personal protective equipment) and correct OSHA guidelines. Must be able to stand and sit for long periods of time. Ability to tolerate heavy use of computers. Must be able to commute via Tram when required. Ability to lift 10-20 lbs. Assist with transferring patients using proper body mechanics from wheel chair, stretcher, and exam tables as required.
