<td align="left"><font style="font-size: 100%;">Infusion Pharmacist - Relief<BR> <BR>Department Pharmacy Services<BR> <BR>Location CHH 1st Floor<BR> <BR>Work Unit Description <BR> <BR> <BR> <BR>Work Schedule, Hours, FTE, Salary Range <BR> <BR> FTE: 0.03, Schedule: Variable, Salary: $45.66 - $62.05/hour, Location: Marquam Hill.<BR> <BR>Functions/Duties of Position <BR> <BR> This purpose of this position is to provide the services of a licensed pharmacist responsible for the safe and proper distribution to and use of medications by patients in the Center for Health and Healing Infusion Pharmacy (CHHIP), Research Pharmacy Services (RPS), Community Hematology/Oncology (CHO) and Doernbecher Infusion Pharmacy (DIP). Patients may range in age from neonates, infants <1 year old, children and adolescents ages 1-18, adults ages 19-65, and the elderly age >65. This position is to function at higher decision making levels regarding drug therapy, proactively affecting drug use by making recommendations at the time prescribing decisions are being made or, in some cases, making independent decisions about drug therapy in cooperation with other health team members. Included in this is the responsibility of managing Research Studies in conjunction with our Research Pharmacy team. Out of the CHHIP, the pharmacist will be responsible for the oversight of the medications prepared and distributed to the Outpatient Treatment Unit, operating rooms and other clinics in the Center for Health and Healing. In the DIP, the pharmacist will be responsible for patients in the Center for Hamatalogical Malignancies, Immunology Infusion Unit and Doernbecher Hematology/Oncology patients. This position is responsible for the direct oversight of the day-to-day operations in the respective pharmacy (CHHIP/DIP), as well as ensuring that the pharmacy is in compliance with regulatory, organizational and departmental standards and policies.<BR> <BR>Job Requirements <BR> <BR> Required: BS in Pharmacy or Pharm.D. PLUS Practice Residency or Specialist Certification (BCPS, BCOPS, etc) or Equivalent Experience. A current Pharmacist?s license issued by the Oregon State Board of Pharmacy. OHSU may require training or experience in a specialized area at time of recruitment. Experience in Oncology pharmacy practice, either inpatient or outpatient. Inpatient and/or outpatient hospital pharmacy practice. Oncology related drug therapy in patients described in Position Purpose. (Dosage, indications, side effects, etc. of chemotherapy and oncology related adjunctive medications). Contemporary medication use systems (interpret prescribers orders, prepare and dispense medications and sterile products, etc.). Interpersonal communication, formal and informal communication, including presentations. Project management concepts. Systematic problem solving. Computer skills including use of word processing, spreadsheet, and databases. Patient interviewing and teaching. Problem solving. Preferred: PGY-2 Specialty Residency. BLS/ACLS certification. Vaccination certification. Specialist Certifications (BCPS, BCOPS, etc </font></td>
