<td align="left"><font style="font-size: 100%;">Title: Nurse Practitioner - Connections<BR>Location: Oregon-Portland<BR>Job Number:_15004<BR>Providence is calling a full-time Nurse Practitioner-Connections for a day shift position at Providence Portland Medical Center, and other sites as necessary, in Portland, OR. <BR><BR>In this position, you will:<BR><BR><BR><BR>* Provide clinical expertise within Connections Palliative Care to improve patient care for patients with complex or life threatening illnesses.<BR>* Build effective relationships with referring physicians to assure coordinated care and improve the culture of collaboration among specialists, primary care physicians, nurses and other health care providers to meet the needs of the patient.<BR>* Also provide leadership in the development of end of life care and the increased awareness and understanding of the palliative care services of Providence Health & Services - Oregon.<BR><BR><BR><BR>Qualifications:<BR><BR>Required qualifications for this position include: <BR><BR><BR>* Masters prepared Nurse Practitioner graduate.<BR>* Current license in the State of Oregon as a Certified Adult or Family Nurse Practitioner.<BR>* Oregon controlled substance license required.<BR>* Current BLS (American Association Basic Life Support for Health Care Provider) required within 30 days of hire.<BR>* Valid driver?s license, automobile liability insurance.<BR>* Documented continuing education in palliative care.<BR>* Assigned Federal Drug Administration number.<BR>* In compliance with the Providence Health & Services-OR Human Resources policy #302 Background Checks?, a criminal background check is required for this position, based on assigned duties.<BR><BR><BR>Preferred qualifications for this position include:<BR><BR>* Certified Hospice and Palliative Care Nurse Designation preferred.<BR>* Five years experience in a setting dealing with terminally ill patients such as oncology, geriatrics, clinic practice or hospice preferred.<BR>* Experience in the tools and techniques of quality improvement preferred.<BR><BR><BR>About Providence in Oregon<BR><BR>As the largest health care system and largest private employer in Oregon, Providence offers exceptional work environments and unparalleled career opportunities.<BR><BR>The Providence Experience begins each time our patients or their families have an encounter with a Providence team member and continues throughout their visit or stay.
