               <td align="left"><font style="font-size: 100%;">Title: Nurse Practitioner-Hospice<BR>Location: Oregon-Portland<BR>Job Number:_17954<BR>Providence is calling an on-call Nurse Practitioner for a rotating/variable shift position at Providence Home Services Building in Portland, OR. <BR><BR>In this position, you will:<BR><BR>* Perform history and physical examinations to assess physical, psychosocial, emotional and spiritual needs of the patient.<BR>* Assess the information and collaborates with the team to develop a plan of care which includes the needs of the patient, family and caregivers.<BR>* Provide training and continuing education to hospital/Hospice staff, physicians, ancillary departments, contracted facilities and students.<BR>* Evaluate current practice and assists in developing policies, procedures and protocols to improve quality of care.<BR>* Work in collaboration with the Interdisciplinary Team, Medical Director, community physicians and hospital staff.<BR><BR>Qualifications:<BR><BR><BR>Required qualifications for this position include:<BR><BR>* Graduate of an accredited school of nursing.
