               <td align="left"><font style="font-size: 100%;">Title: Oncology Clinical Pharmacist<BR>Location: Oregon-Portland<BR>Job Number:_19369<BR>Providence is calling a full-time  Oncology Clinical Pharmacist  for a  day shift position at Providence Office Park in Portland, OR.<BR><BR>In this position, you will:<BR><BR>* Be responsible for coordination of medication-related services across Providence Oncology and Hematology Care Clinics. Responsibilities include promotion of evidence-based, safe, effective drug therapy, standardization of medication practices across clinics, and coordination with drug procurement, inventory management and billing processes.<BR><BR>Qualifications:<BR><BR>Required qualifications for this position include:<BR>* Graduate from an accredited College of Pharmacy. Pharm.D. or M.S. degree in pharmacy with residency required unless equivalent experience and education can be documented.
