<td align="left"><font style="font-size: 100%;">Pharmacist/Speciality<BR> <BR>Department Outpatient Pharmacy<BR> <BR>Location SJH 3370<BR> <BR>Work Unit Description <BR> <BR> <BR> <BR>Work Schedule, Hours, FTE, Salary Range <BR> <BR> 1.0 FTE; Schedule: five days a week, days and hours vary; Salary range: $45.66 - $62.05/hour; Location: Marquam Hill<BR> <BR>Functions/Duties of Position <BR> <BR> The purpose of this position is to provide the services of a licensed pharmacist in the Internal Medicine Clinics treating patients with chronic disease states. In collaboration and potentially independently of other health team members, this position is to function at higher decision making levels regarding drug therapy, proactively affecting drug use at the time of prescribing. Responsibilities include the review of effective utilization and facilitation of safe and proper outpatient medication distribution to a patient population who range in age from neonates, infants less than one year of age, children and adolescents ages 10-18, adults ages 19-65 and the elderly over age 65.<BR> <BR>Job Requirements <BR> <BR> A BS in Pharmacy or Pharm.D. Licensure in the State of Oregon. State of Oregon Preceptor License preferred. Clinical pharmacy practice experience to include active participation on chronic care teams; identify, resolve and document resolve complex theraputic problems; experience utilizing evidence based literature. Pharmaceutical care principles. Disease related modification of drug therapy. Project management skills and experience in contermporary medication use systems. Strong interpersonal communication skills. Systematic problem solving skills. Ability to work independantly with minimal supervision. Patient interviewing and teaching skills. Excellent knowledge of third party payors and insurance billing guidelines of Specialty Medications. Computer skills including use of MS-Word, Excel and Access databases. Sucessfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance and OHSU policies. Preferred: Advanced Pharmacy Practice Residency, BCPS- OR- five (5) years of equivalent experience. Experience in clinical and/or business program development. Direct patient care or medication therapy management experience.<BR> <BR>Additional Details <BR> <BR> The pharmacist will mimic the hours of the Internal Medicine Clinic, rotating between clinic locations. Hours will be daytime hours, Monday through Friday with the potential for Saturday daytime hours. The pharmacist will experience competing demands from patients, nurses, physicians, pharmacy staff and other healthcare professionals, as they attempt to influence clinical decision-making, clinical policies and practices and work flow. The pharmacist will have direct contact with patients and must be able to provide care and communicate effectively in potentially difficult, uncomfortable and intense situations on a daily basis.
